To signup for one of our Individual Leagues, please do the following:
  1. Print this form.
  2. Enter your name, address, telephone numbers, and Email address.
  3. Select the Fax, Mail, or Internet delivery.
  4. Read and sign the agreement.
  5. Mail the completed and signed form with a check for $40 to the address below.
  6. Your balance: $50 for Fax teams, and $25 for Mail Teams (Internet Teams are paid in full by the above $40 deposit) is due on opening day.

    Agreement

    By signing the AGREEMENT, the undersigned agress to have Precision Sports Statistics Inc. provide Stat League statistics the 2006 baseball regular season. In addition, the undersigned agrees that Precision Sports Statistics Inc's liability is limited to only the amounts remitted in payment for said services.
