Here is a copy of our 2005 Rotisserie-style football order form. Complete both pages of this form and return it with a check or money order for $30 to:

Your customized draft package will be sent within 2 days of receipt of your completed order form and deposit.

If you are unable to print either of these order forms clearly, we need the following information: Your name, mailing address, and phone number.
The expected number of teams, active and reserve roster sizes for your league.
The NFL teams eligible for your league (usually entire NFL).
The # of players at each position that you will draft per team.
The scoring values for each of the scoring categories you want to use.
A name for your league, and your choice of delivery: Email, US mail or fax.
Be sure to specify that you have a "Rotisserie-Style" league.
In this case a clear copy of a signup form will be sent to you with your draft package.
