To signup for our our Internet Only Basketball League service, please do the following:
  1. Print this form.
  2. Enter your name, address, telephone numbers, and Email address.
  3. If you used our service last year for basketball, please include your 2000-2001 league #.
  4. Read and sign the agreement.
  5. Mail the completed and signed form with a check for $30 to the address below.

Make sure the email address is printed clearly. Your league number and commissioner password will be emailed to you upon receipt of this order form and deposit. You will be able to setup your league, print out your customized draft package, enter your draft, and get your first statistics using this league number and password.

The price is $24/team, less the $30 deposit. The balance is due within 7 days of your first stat report.

Agreement

By signing the AGREEMENT, the undersigned agress to have Precision Sports Statistics Inc. provide Stat League statistics the 2001/02 basketball regular season. In addition, the undersigned agrees that Precision Sports Statistics Inc's liability is limited to only the amounts remitted in payment for said services.
