Here is a copy of our 2001/02 basketball order form. Complete both pages of this form and return it with a check or money order for $30 to:

Your customized draft package will be sent within 2 days of receipt of your completed order form and deposit.
If you are unable to print either of these order forms clearly, we need the following information:
Your name, mailing address, and phone number.
The expected number of teams, active and reserve roster sizes for your league.
The NBA teams eligible for your league.
The scoring categories you want to use.
A name for your league, and your choice of delivery: Email, US mail or fax.
If this form does not print clearly on your printer, please contact us and we will fax you one.
